Television talk show host Jimmy Fallon returned to The Tonight Show this week following a two-week medical leave after severely injuring his ring finger. The injury – called ring avulsion – occurred when Fallon tripped on a rug at home and snagged his ring on a countertop as he fell. The digit was bent completely sideways and required six hours of intricate surgery, including a vein transplant from his foot. Doctors were able to save the finger. Although Fallon made the injury seem like it wasn’t a big deal, ring avulsion can be serious, resulting in possible amputation of the finger. Here’s what happened. “My ring got caught on the countertop, got stuck there, and pulled my finger off!” explained Fallon during his Monday evening broadcast. “I’m getting up and my finger is sideways. It looks completely fake, like a cheap horror movie where you see a broken finger – that’s what it looks like.” The Three Grades of Ring Avulsion “When a standard wedding ring is caught on something, chances are the skin and tissue will fail long before the ring,” said Bill Gowski, MD, a hand and microvascular surgeon at TOSH – The Orthopedic Specialty Hospital in Murray, Utah. “In a ring avulsion, the ring acts like a tourniquet and pulls tissue or skin off the finger. The injury can range from discoloration of the finger due to internal damage of the tissue, or complete removal of skin and tissue from the bone, known as de-gloving,” added Dr. Gowski. Low Grade – The finger received some trauma and may experience swelling. However, there is blood flow to the finger, so treatment typically involves placing a splint and letting the injury heal. Medium Grade – During the fall, the artery in the finger was stretched and the inner layer of the artery is damaged, causing the blood to clot and restrict blood flow into the finger. Treatment would require an artery graft to replace the damaged one and restore blood flow to the finger. High Grade – The ring may actually de-glove the finger bone, removing skin and tissue completely off of the bone. In this instance, amputation is highly likely. Life After a Ring Avulsion Injury If you suffer a ring avulsion, you should go to the emergency room so the injury can be evaluated and graded appropriately. You may be referred to a hand specialist who can evaluate and provide the best treatment option based on the grade of the injury. People who suffer a medium grade injury should get back 80% of the sensation in the finger, but physical therapy may be required to get back the full range of motion in the finger. “In many cases, having a stiff finger long term is a likely outcome,” said Dr Gowski. “Jimmy may have trouble playing the guitar like he used to. The extent of recovery is typically dependent upon the severity of the initial injury. With therapy and the passage of time, he may be able to get the finger back to normal, but at the very least I would expect some minor deficits in motion and/or sensation.”

Fallon explains his disappearance from the show stems from an injury called ring avulsion, a rather graphic injury that Fallon warns anyone without a penchant for gruesome images not to Google. Ring avulsion is actually a spectrum of injuries that can range from contusion to amputation, and judging from Fallon’s description of what happened to him, his finger found itself on the more deadly end of the spectrum.

As it turns out, his doctor truly was a savior, since most cases of ring avulsion often end up in having the finger cut off. His doctor successfully completed the surgery under a microscope, taking a vein from Fallon’s foot and transferring it to his finger. Still, Fallon had to spend 10 days in the intensive care unit, reading up on the meaning of life, specifically Victor E. Frankl’s “Man’s Search for Meaning,” and watching The Duff, what Fallon says to be the Mean Girls of 2015, three times.

Fallon says he wrapped his hand in a bath towel and rushed to the Beth Israel E.R., where he was advised to go to the Bellevue Hospital in New York City instead because he would need microsurgery to fix his finger. There, he describes his doctor as a bow tie and cowboy boot-donning man and how, the minute Fallon saw him, he knew he was going to be his finger’s savior.

But alas, I am not a professional, and so, I decided to ask one. And Rich Goldberg, president of  Safian & Rudolph on Philly’s Jewelers’ Row confirms my suspicions: “Usually when a ring is catching in such a manner, it is too large and should be sized down,” he says. “Many men are not used to wearing a ring or wedding band and opt for a larger size, thinking it will be more comfortable, but a snug fit is always recommended. And anyway, in a week or two you most likely won’t think twice about your ring because you’ll get used to how it feels.” (He also recommends taking your ring off when you’re taking part in any type of activity where it might snag something, but it doesn’t sound like such a precaution would have even helped poor Jimmy.)

Fallon, 40, said that his wedding ring got caught on the countertop, got stuck there when he was falling and pulled his finger off. He then wrapped his hand in a cheap towel, got in a cab and headed to the emergency room.

Fallon’s doctors may have been able to reattach his finger by replacing the crushed vein with one from his foot, but in most ring avulsion cases, amputation is the safest option. A 2008 study from the Journal of Hand Surgery says that new microsurgical techniques have led to a rise in successful finger reattachments and repairs but it also noted that “amputation is still the best option for most patients.”

“To the nurses and doctors over at Bellevue Hospital, thank you so much for taking care of me. I love you guys,” Fallon says. “It’s a tough, tough job. You guys are rock stars and just awesome, awesome people.”

As for his wedding ring, it too got deformed during the fall and is now back at the jeweler’s for repair. However, after what happened to his finger, the comedian says he loves his wife but is now scared out of his wits to put the ring back on.
